Community Nursing Care

Community Nursing Care is a specialised healthcare service that provides support and care for individuals with disabilities in their homes or within their community. Our goal through this service is to improve the quality of life and promote independence for individuals with disabilities by addressing their physical, emotional, social, and psychological needs. Our nurses work closely with participants and their families to develop a care plan that is tailored to them, including medication management, wound care, pain management, and other healthcare services. This service is provided by our highly trained and experienced team who are committed to providing compassionate, participant-centred care.

Supported Independent Living

Our Supported Independent Living (SIL) service is aimed at assisting individuals with a disability to live as independently as possible in their own homes or in shared accommodation. SIL provides support in areas such as personal care, household tasks, cooking, shopping, and transport. The level of support provided is based on the individual’s needs, goals, and preferences, and is designed to help them achieve greater independence and participation in their community. This service is delivered by our team of support workers who work closely with the individual and their family to develop a personalised care plan.

Assistance with Daily Personal Activities

At Ability Care Connect, we understand that daily personal activities can be challenging for individuals living with disabilities. That’s why we offer Assistance with Daily Personal Activities, a type of disability support service that provides compassionate and respectful assistance to individuals who require help with their daily activities. Our highly trained professionals work to ensure that our participants personal care needs are met, enabling them to live as independently as possible either in their own home or in a residential care setting. That’s why we offer Support Coordination, a service that empowers individuals with disabilities to make informed choices about the support they need. Our experienced support coordinators work with individuals to develop a personalised support plan that is tailored to their unique needs and preferences. This may involve finding and accessing appropriate services, coordinating appointments, and liaising with service providers. Our focus is on building capacity and promoting independence, so individuals can fully participate in their communities.

Community Participation

Community Participation is a vital aspect of our disability services, which aim to enable individuals with disabilities to participate actively and fully in their local communities. Assistance with Travel and Transport is one of our disability services that is designed to help individuals with disabilities to access transport and travel safely and comfortably. This service may involve assistance with planning and organising travel, as well as providing support during the journey. We understand that travelling can be challenging for individuals with disabilities, and our aim is to ensure that they can access the transport they need to participate fully in their community. Our trained professionals are compassionate and respectful, and work with the individual to identify their unique needs and preferences, and to provide the support required to make travel as smooth and stress-free as possible.
